As a band, we have discussed this and collectively decided that before we audition anyone, we feel that we need to get to know you better. That is why we devised this clever application for you to complete prior to your audition.

Also, please include a 500 word essay on one of the following prompts (Write at the top of the paper which prompt you are addressing):
1. Evaluate a significant experience, achievement, risk you have taken, or ethical dilemma you have faced and its impact on you.
2. Discuss some issue of personal, local, national, or international concern and its importance to you.
3. Identify and discuss a significant problem facing your generation.
4. Topic of your choice, for example, a descriptive essay of personal meaning to you.
