Membership Form & Release of Liability
The membership in the Gold Strike Girls club includes but it not limited to social gatherings, business promotion, and advertising. I am joining this club at my own risk and agree to hold harmless any of the clubs affiliates, agents, etc. I understand that the clubs officers and/or members may revoke my membership at any time for any behavoir deemed unappropriate. I agree to adhere to the bylaws, code of ethics and represent the Gold Strike Girls in a socially acceptable manner.
Image Release: For good and valuable Consideration herein acknowledged as received, and by signing this release I hereby give the Club/Photographer/Members/Agents and Assigns my permission to license, distribute and promote the use of Images in any Media for any purpose which may include, among others, advertising, promotion, marketing and packaging for any product or service. I agree that the images may be combined with other images, text and graphics, cropped, altered or modified. I acknowledge and agree that I have consented to publication of my first name and hometown as indicated below for descriptive purposes.
I agree that I have no rights to the images, and all right to the Images belong to the Photographer and assigns. I acknowledge and agree that I have no further right to additional consideration or accounting, and that I will make no further claim or any reason to Photographer or assigns. I acknowledge and agree that this release is irrevocable, worldwide and perpetual, and will be governed by the laws of the United States of America.
I represent and warrant that I am at least 18 years of age, have provided a valid copy of identification and have the full legal capacity to execute this release. I hereby release, waive, discharge and covenant not to sue 4 Leaf Photography & Promotions, its officers, servants, agents and employees (hereinafter referred to as "releases") from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or relating to any loss, damage or injury, including death, that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the releasees, or otherwise, while participating in, or while in, on or upon the premises where the Shoots will be conducted, while in transit to or from the premises, or in any place or places connected with the Shoot. I am fully aware of risks and hazards connected with being on the premises and participating in the shoot, and I am fully aware that there may be risks and hazards unknown to me connected with being on the premises and participating in the shoot, and I hereby elect to voluntarily participate in the shoot, to enter upon the premises and engage in activities knowing that conditions may be hazardous, or may become hazardous or dangerous to me and my property. I voluntarily assume full responsibility for any risks of loss, property damage or personal injury, including death, that may be sustained by me, or any loss or damage to property owned by me, as a result of my being a participant in the shoot, whether caused by the negligence of releasees or otherwise.
I further hereby agree to indemnify and save and hold harmless the releasees and each of them, from any loss, liability, damage or costs they may incur due to my participation in the club, whether caused by the negligence of any or all of the releasees, or otherwise. It is my express intent that this Release shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representative, if I am deceased, and shall be deemed as a Release, Waiver, Discharge and Covenant Not to Sue the above named releasees.

In signing this release, I acknowledge and represent that:
A. I have read the foregoing release, understand it, and sign it voluntarily as my own free act and deed;
B. No oral representation, statements or inducements, apart from the foregoing written agreement, have been made;
C. I am at least eighteen (18) years of age and fully competent; and I execute this Release for full, adequate and complete consideration fully intending to be bound by same.
Definitions:
Model means me and includes my appearance, likeness and form.
Media means all media including digital, electronic, print, television, film and other media now known or to be invented.
Photographer means photographer, illustrator, filmmaker, or cinematographer, or any other person or entity photographing me.
Assigns means a person or company to whom photographer has assigned or licensed rights under this release as well as licensees of any such person or company.
Images mean all photographs taken of me.
Consideration means something of value I have received in exchange for the rights granted by me in this release.
Shoot means the photographic or promotional session.
