Bus Transportation:   $8.00
Buses leave at 7 a.m. and return at 6 p. m. both days.  Locations will be posted by June 26, 2009.
 
DAY 2    Ages 13 -  17/18 (in High School)
 
When:    Saturday, July  25, 2009
Place:      Southeastern Louisiana University
 
Time:      8:00 a.m. until 4:00 p.m.
Cost:       $125.00
Bus Transportation:   $8.00
There is an additional $3.00 charge to use PAYPAL.
Curtis Baham’s Special Events” Celebrity Pro Football / Cheerleading Camp
Due to the popularity of our camp; each year it becomes larger.  We have added a second day in order to accommodate the area; however, space is limited.  Once we have met our limit, unfortunately we will not be able to take any other applicants.  Don’t miss out on this exciting opportunity every child deserves.  Register early to assure your space.  Those needing transportation need to be paid by July 10, 2009 to assure a seat on the buses.
Football Players will work with children on there agility, speed, passing, drills, routes, running, one on one, catching, and combine drills (for advance players).
 
Cheerleaders will work with children on drills, cheers, jumps, and more.  This event will enable our youth to understand what it takes to accomplish their goals, and will give them the opportunity to work side by side with athletes who have succeeded in the cheerleading profession.
 
Children will have the chance to get autographs from the NFL players and USA professional cheerleaders.  Giveaways, food, drinks, T-shirts, and certificates will be provided for each child.
 
We encourage parents to bring a chair and spend some time with us.  There will be tents set up by sponsors, pictures, football/cheerleader items for sale, as well as food and beverages for sale.
 
NOTE:
Please  be sure to wear gym clothing (sweats, short, tights, tee shirts) and tennis shoes/ football cleats.

I hereby give my child _________________________________________ permission to participate in the “Curtis Baham’s Celebrity Football Camp”.  I understand that I will be fully responsible for any medical emergencies that my child may need during this camp, and by signing this form, I free the camp from all expenses in case of injury.  I also give the temporary authority of any medical attention needed for my child to the paramedics until I can be contacted of any injuries my child may have.

I would like my child ________________________________________ to ride the bus to the camp for an additional $8.00 dollars.  I understand that I must sign the BUS ATTENDANCE SHEET in order for my child to get on the bus that will leave at 7:00 a.m.; And I also understand that I must sign my child out once he/she gets off the bus at 6:00 p.m.  I understand that Curtis Baham’s Special Events is providing transportation by an outside licensed bus company and is not liable for any injuries that can occur to and from the camp.  By signing below, I relieve Curtis Baham’s Special Events of any and all liabilities that may occur on any of the buses.
