I use these 3 forms the most.

1. Customer information sheet
2. Copyright Release form
3. Model/ Customer Release Form

Here is a link to all the forms you should need for photography releases and such. Just print them off, they are pretty simple yet thorough! Hope this helps!

Go to Books-a-million and purchase the book "Business and Legal Forms for Photographers. It comes with a CD of all the forms that is really easily uploaded to your computer.
This book is invaluable when it comes to keeping things legal.
